August Underground's Mordum, for pretty much the same reasons Dissident said for his choice. First time I saw it was when one of my friends showed me a clip from it, and not realizing it wasn't real I couldn't even bring myself to watch the whole thing.  Once I realized it was just special effects it wasn't as bad, but still incredibly disturbing because of how convincing it is, and if I had willingly suspended my disbelief it would easily be the most fetched up thing in the history of the universe.

As far as actually being scary, can't say I've seen much that actually spooked me but that's probably just because I don't watch that many horror movies.

The White Ribbon is probably the last movie I saw that made me feel pretty bad afterwards, but I have never found a movie "jump scary".  The White Ribbon is just a pretty depressing, horrific drama movie.

A movie that I've only seen a miniscule fraction of is: Slaughtered Vomit Dolls. It looks creepy in a sex depraved way, hence, I'm too not looking to watch it.

Another movie I don't want to watch is: Philosophy of a Knife. Japanese conducting experiments on human victims (torture) in WWII, supposed to be based on factual evidence.
